A MavenAGI-managed authentication credential used by the Microsoft Teams integration expired, causing authentication failures and preventing the integration from processing requests normally. Once the issue was identified, the credential was rotated and service was restored.
The incident was caused by the expiration of a credential required to authenticate with the Microsoft platform.
The credential expired without an effective advance notification reaching the appropriate operational channels. In addition, the process for managing and rotating the credential relied on restricted administrative access, which increased the operational dependency associated with renewal.
These gaps meant the credential was not rotated before expiration and the issue was not identified until the integration failure was reported.
The expired credential was replaced with a newly generated credential and the integration was redeployed. Authentication was restored following the update.
We are improving our credential management process to reduce the likelihood and impact of similar issues. This work includes:
Adding proactive monitoring and reminders for credential expiration.
Establishing a documented playbook for credential rotation and recovery.
Improving operational access and recovery procedures for credential management.
Periodically auditing managed credentials to identify upcoming expirations.
Reviewing ownership and operational coverage for externally authenticated integrations.
